Systemic infection and brain injury in the preterm infant.

Neste número do Jornal de Pediatria, Silveira et al. investigaram fatores de risco para o desenvolvimento de leucomalácia periventricular (LPV) emuma coorte de recém-nascidos de muito baixo peso (RNMBP). Um número significativamentemaior de recém-nascidos comLPVapresentaramsepse documentada e/ou foram mantidos sob ventilação por mais de 24 horas. Esse estudo bem delineado acrescenta evidência de suporte ao papel da infecção pós-natal na patogênese da LPV. Ao passo que os dados são provocativos, não está claro pelo artigo como o período de episódios de sepse está relacionado ao desenvolvimento de LPV. Se todos os recémnascidos do estudo tiveram sepse de início precoce, é de certa forma surpreendente que os estafilococos coagulase-negativos tenhamsidoospatógenos mais freqüentemente encontrados. Nos EUA, esse microorganismo seria uma causa rara de sepse de início precoce. Se os recém-nascidos do estudo tiveram infecções de início tardio, a questão do tempo de instalação do episódio infeccioso com o início de LPV é especialmente importante. Uma grande limitação desse estudo é a falta de dados sobre a probabilidade de infecção antenatal. Essa questão é particularmente importante, dada a alta taxa de bacteremia por Mycoplasma e Ureaplasma em recém-nascidos prematuros. Uma terceira deficiência desse estudo (conformeobservado pelos autores) é a falta de informações obtidas por ressonância magnética (RM); a RM é umamaneira mais sensível de diagnosticar LPV. Dadas essas limitações, entretanto, os autores devem ser congratulados por abordarem essa questão.
